What is Support Coordination under the NDIS?

Support Coordination is a vital service that assists participants in understanding and utilizing their plans effectively. It is funded at varying levels, depending on the individual's needs for managing services and achieving their goals through capacity building services.  

  

🔹 Level 1: Support Connection  

  

- Explaining your NDIS plan in simple terms  

- Helping you find and connect with providers  

- Building your capacity to manage supports independently and navigate the NDIS  

- Setting up initial services (e.g., therapy, personal care)  

  

🔹 Level 2: Coordination of Supports (Support Coordination)  

  

- Developing a mix of supports tailored to your needs  

- Coordinating multiple providers (e.g., therapists, support workers)  

- Troubleshooting issues or service gaps  

- Assisting in preparation for plan reviews  

- Building skills to gradually increase independence  

- Monitoring the effectiveness of your supports  

  

🔹 Level 3: Specialist Support Coordination  

  

- Addressing complex challenges (e.g., housing instability, health risks, justice involvement)  

- Crisis planning and response  

- Coordinating across multiple systems (health, housing, mental health, etc.)  

- Reducing risks, including conducting physical and mental health assessments, using experience in completing a range of assessments like FCAs, WHODAS, and K10s.  

- Designing long-term support strategies for your physical and mental health recovery.
